Please complete and include this nomination form which identifies your candidate for an Award of Excellence from the Fish and Game Commission. The nomination may be made posthumously.
Be brief, but as complete as possible. Please answer the following questions:
- What did the person or organization do to meet or qualify them for this distinction?
- What organization/groups/location did the nominee impact and how?
- If you are nominating an individual, please be sure to tell us enough about the individual’s service to the community, memberships in conservation organizations, and special contributions to support your nomination.
- Include testimonials and ancillary materials as separate documents in an email to Tanya.haskell@wildlife.nh.gov. In the subject line please include the name of the Organization or Individual you are nominating. Or mail one package that includes everything you want the Commission to review. Do not exceed 5 (five) typewritten pages of material.
